What Type Of Infection Is This?

im so sorry i had a logn detailed post and answered all the questions abt parameters and a VERY long story about what happened and how it happened..but as i was adding the picture i clicked backspace and i lost everything...its really late, i was wondering if any of you can just help me identify what type of infection killed my fish?

within 2 days of first cloudy eye(if thats what it even was) symptoms as it got progressively worse very quickly, until now when my goldfish just died.

what type of infection is this? (one other oranda was completely unaffected and is still totally fine)

The guy on the pics passed? Usually cloudy eye is associated to water problems.

So if you could fill out these quick:

[*]Test Results for the Following:

[*]Ammonia Level?

[*]Nitrite Level?

[*]Nitrate level?

[*]Ph Level, Tank (If possible, KH, GH and chloramines)?

[*]Ph Level, Tap (If possible, KH, GH and chloramines)?

[*]Brand of test-kit used and whether strips or drops?

[*]Water temperature?

[*]Tank size (how many gals.) and how long has it been running?

[*]What is the name and "size of the filter"(s)?

[*]How often do you change the water and how much?
