female bn pleco

On Saturday I bought a female bn pleco and suddenly she has this white fungus stuff on her back and ripped fins. I have moved her to her own tank and will fill in the form tomorrow and test the water more precisely with my new API master water test kit and does anyone know what my female bn pleco might have

Would someone be able to copy the form for me as I am on a kindle in there next post she was also by herself in a tank so no I didn't quarantine her but I think my male bn May have done some of the damage

    • * Ammonia Level (Tap):
    • * Nitrite Level (Tap)

    • * Ph Level, (Tank) (If possible, KH, GH and chloramines)
    • * Ph Level, (Tap) (If possible, KH, GH and chloramines)
    • Tank information:
      • * Brand of test kit (drops or strips):
      • * Filter name and size (GPH/LPH)
      • * Additives and conditioners:
      • * Water temperature (With or without heater- please specify):
      • * Last water change date:
      • * Tank size (Gallons or litres):
      • * Fish in the tank (species and number):
  • * Any new fish added into the tank:
  • -If Yes: Were they quarantined?
  • -If Yes: Quarantine period and process (meds added):
  • * List prior health issues in tank:
  • * Medications previously added to the tank:
  • * Medications on hand:
  • Affected fish information:
  • * Diet- brand and amount/duration of feeding:
  • * Any unusual behaviour such as nipping, bottom sitting, etc:
  • * Any unusual findings on fish and in tank:
  • * Affected fish in quarantine or in main tank:
  • * Current medications being used on affected fish:
  • * Previous medications used to treat affected fish:
  • * If any, list previous health issues and treatment protocols taken:)
  • * You can really help us to identify with the concern more accurately if you post some pictures and a short video.

Though that may have been the case, this is why we quarantine any and all new fish we bring home. Please do make sure to watch your man tank as well, so we can make sure nobody else catches any of the problems this fish has developed. :hug
